If you are interested in donating an item for the 2008 HR SOUTHWEST Silent Auction, please complete the form below and fax it to (214) 631-4533.  All prizes must be received at the office of Dallas HR Management Association, Inc., 4100 Spring Valley Road, Suite 300, Dallas, TX 75244 by Friday, September 26, 2008 .  Please clearly mark “Silent Auction donation” on the outside of the package, and include your company’s name, address and a contact person.  If mailing the item is not feasible, please contact the HR SOUTHWEST office at (800) 644-7476, ext 22 to make alternate arrangements.
